Child Immunization and School Clinics

To submit or view immunization information using ICON you will need:

Option 1: Ontario Health Card (OHIP)

• Immunization record or supporting documents.
• Ontario Health Card.

Option 2: Ontario Immunization Identification ID (OIID) Number

• Immunization record or supporting documents.
• Ontario Immunization ID found on the top right-hand corner of the Durham Region Health Department immunization letter or notice.
• Ontario Health Card number (OHIP): You only need this the first time to set up a unique PIN. If you don’t have OHIP, call us.
• Unique PIN.

Disclaimer: Immunizations on file at the Health Department may only be as accurate as what we have received. Please submit any missing immunizations to ensure we have a complete record. Include photos of the records or supporting documents.

Note: It may take up to ten business days before any updates appear on your child’s record when you use Immunization Connect Ontario ICON.

Two months of age (Three immunizations due)

Doses of MMR vaccine, Men-C-C vaccine and Pneu-C-15 vaccine are needed on or after age one regardless of previous doses.

15 months of age (One immunization due)

• Var | Varicella (chickenpox) vaccine

18 months of age (One immunization due)

• DTaP-IPV-Hib | Diphtheria, tetanus, pertussis, polio, Haemophilus influenzae type b vaccine

Four years of age (Two immunizations due)

• Tdap-IPV | Tetanus, diphtheria, pertussis and polio vaccine (e.g. Adacel-Polio ®)
• MMRV | Measles, mumps, rubella and varicella vaccine (e.g. Priorix-Tetra ®, ProQuad ®)

A dose of Tdap-IPV vaccine is needed on or after four years of age regardless of previous doses.

Grade 7 vaccines
(Report online ONLY if received somewhere other than a Durham Region Health Department clinic, e.g. school or catch-up)

• Men-C-ACYW135 | Meningococcal conjugate type ACYW135 vaccine (e.g. Menveo ®, Menactra ®, Nimenrix ®)

14 - 16 year old booster (ten year booster) - Most common missing vaccine for high school students

• Tdap | Tetanus, diphtheria, pertussis vaccine (e.g. Adacel ®, Boostrix ®)

• Use our online reporting form
• Fax the record to the Durham Health Connection Line at 905-666-6216.
• Mail a copy of the record to:

• Hand in a copy of the record to your child care centre.
• Call the Durham Health Connection Line.

The best way to access your child’s record is using the online tool Immunization Connect Ontario (ICON).

To view or print your child’s record on file with the Health Department using ICON, you will need to create an account and set a PIN with your child’s Ontario Immunization ID (OIID) and Ontario Health Card.

• If you are not able to set up your PIN online, please call 1-800-841-2729 to get help creating your account.
• You can also call the Durham Region Health Connection Line to request a copy of the immunization record.

Durham Region Health Department gives up to three vaccines to grade 7 students. Eligible grade 8 students who missed the vaccine(s) in grade 7 can get caught up at school clinics.

The Health Department will be offering Human Papilloma Virus (HPV), Hepatitis B (Hep B), and Meningococcal C-ACYW-135 vaccines at our school-based immunization clinics.

• Human Papillomavirus (HPV) (2 doses, 6 months apart)
• Hepatitis B (2 doses, 6 months apart)
• Meningococcal C-ACYW-135 (Men C-ACYW-135) (one dose only)

Grade 7 students will be given a consent package at the beginning of the school year. Review this package with your child, complete the consent form and return it to the school at least 2 weeks before your fall clinic. If your grade 7 or 8 child needs another copy of the consent package, please ask your school secretary.

Clinics will be taking place in the fall and spring. Find out the clinic dates for your child's school using the table below. Dates may change throughout the year.

Before the vaccination, let your child know:

• What to expect.
• Why the vaccines are important.
• That the needle might feel like a pinch in the upper arm that only lasts a few seconds.
• That the nurse will help to decrease their fear/pain (e.g.: distraction, deep breathing, relaxing their arm, talking them through the experience etc.).

On the day of the vaccination, have your child:

• Eat something before their appointment.
• Wear a shirt with loose sleeves.
• Wear or bring a mask (may be required as per current public health requirements).
• Follow instructions provided by the public health nurse and your reminder email to access the clinic.

If your child is nervous about getting needles, use the CARD system to have a more positive vaccination experience.

If your child missed getting the vaccine(s) at school he/she may get it at their next school-based clinic. If you have questions, you can call the Durham Health Connection Line.

Every year, the Health Department reviews immunization records of students to ensure they are up-to-date for required vaccines. Our goal is to ensure students are immunized to help keep them healthy and avoid the spread of vaccine preventable diseases, while meeting requirements to avoid future suspension. Health care providers are not required to update the Health Department with vaccines your child has received. We rely on parents to keep us updated.

If you received an immunization reminder letter from us, we are missing vaccine information for your child.

Here is what you should do next:

• Talk to your health care provider to find out if your child is up to date.
• Compare the record on the letter with your health care provider’s record.
• If your child needs vaccines, see if you can make an appointment with your health care provider. You can also make a plan with your health care provider to receive any missing vaccines in the future. If you don’t have one, a health care provider, try calling walk-in clinics in your area as some of them may be able to give your child the missing vaccines. Bring the letter with you to your appointment.
• Give us the update for the missing vaccines.

All students under 18 years of age need vaccinations for the following diseases, as required by the Immunization of School Pupils Act (ISPA):

• Tetanus
• Diphtheria
• Pertussis
• Polio
• Measles
• Mumps
• Rubella
• Varicella (if born on or after January 1, 2010)
• Meningococcal disease group C (under 12 years of age)
• Meningococcal disease group ACYW-135 (over 12 years of age)

Medical exemptions

If your child cannot get vaccines due to medical reasons, download the Statement of Medical Exemption - ISPA form. This form must be completed by a doctor or nurse practitioner.
