PASS Case Management event to highlight community resources

Whitby, Ontario – The Region of Durham, in partnership with Community Living Oshawa/Clarington, is hosting a Prompt Access to Single Session (PASS) Case Management event where adults (over the age of 18) with a developmental disability and/or their caregivers can speak directly with an experienced and knowledgeable case manager. Registration is required to attend.

When: June 25, 2019

Where: Oshawa (location to be provided at the time of registration)

Why: Many adults with a developmental disability, and/or their caregivers, are on wait lists or unaware of the resources available to them. PASS Case Management provides 45-minute, one-on-one sessions with these individuals to connect them to resources, complete forms/applications for funding, and more.

The PASS Case Management event is free to attend. However, registration is required.
