Adults with Developmental Disabilities - Services
Family Services Durham offers Adult Protective Services (APS). These services are for adults who are age 18 or older with diagnosed developmental disabilities. This program provides supports to help people reach their full potential, live as independently as possible and participate in the community.
The Ministry of Community and Social Services funds this program. There is no fee for service.
APS program services
We provide the following services:
- Advocacy
- Help with finding resources to meet basic needs
- Case management
- Service co-ordination
- Support developing strategies for healthy living
- Individual assessment and goal-setting
- Individual and family education
- Life-skills development
- Supportive counselling
- Information and referrals to other resources
We match each person in the program with an Adult Protection Service Worker (APSW). The client and APSW will:
- Work together to identify goals and develop a plan to reach them.
- Meet regularly to work towards these goals.
Services may be short-term or long-term depending on the goals set out. Appointments take place at the client's home or at other locations in the community.
How to access the APS Program
You must apply with Developmental Services Ontario to be in the APS Program. The following people can start the application:
- The person in need of services
- Their family members
- Physicians
- Other community agencies
Developmental Services Ontario will confirm eligibility and identify the needs. They will send the application to APS staff. APS will contact the person.
