Homelessness Coordinated Access System & By-Name List

The By-Name List is a real-time, up to date list of people known to be experiencing homelessness. It allows communities to understand people's unique needs in order to match them to appropriate housing resources. For more information, please see the Coordinated Access Guide.

If you are a Housing Provider and you have a vacancy, complete the following form to notify the By-Name List Coordinator of the vacancy.

If you would like to provide feedback or file a complaint on the Coordinated Access System, By-Name List, or the Homeless Individuals & Families Information System (HIFIS), you can complete the following form.
